Because lithium carries baggage &#8212; decades of association with heavy psychiatric dosing, toxicity monitoring, and side effects that make clinicians cautious. But the story of lithium as a longevity compound has almost nothing to do with psychiatry. Across 18 neighboring municipalities in Japan &#8212; encompassing 1,206,174 people &#8212; they found a statistically significant inverse correlation between lithium concentrations in tap water and all-cause mortality (&#946; = &#8722;0.661, p = 0.003).</p><p>The less lithium in the water, the more people died. The more lithium, the fewer deaths. The worms lived significantly longer (p = 0.047).</p><p>This was not pharmacological lithium at psychiatric doses. y2="14"></line></svg></button></div></div></div></a></figure></div><h2>The UK Biobank Confirmation</h2><p>In 2023, Elisa Araldi and Michael Ristow extended this work with a far larger dataset: the UK Biobank, covering 501,461 individuals, combined with NHS prescription records. Among patients with affective disorders, those treated with lithium showed dramatically lower mortality than those on other medications.</p><p>Multivariate survival analysis identified lithium as the single strongest predictor of increased survival, with a hazard ratio of 0.274 (95% CI [0.119&#8211;0.634], p = 0.0023). In plain language: lithium users had roughly 3.6 times lower probability of death at any given age compared to patients on alternative treatments.</p><p>This was not a lithium-vs-placebo comparison. When overactive, it drives a cascade of pathological processes: tau hyperphosphorylation (a hallmark of Alzheimer&#8217;s), insulin resistance, chronic inflammation, and impaired autophagy &#8212; the cellular cleanup process that declines with age.</p><p>Lithium is a direct inhibitor of GSK-3. At trace doses, it doesn&#8217;t shut the enzyme down &#8212; it modulates it. This gentle inhibition produces effects across multiple organ systems:</p><p><strong>Neuroprotection.</strong> By reducing tau phosphorylation and increasing brain-derived neurotrophic factor (BDNF), lithium supports neuronal survival and plasticity. BDNF is critical for memory formation and cognitive resilience &#8212; and its levels decline with age.</p><p><strong>Anti-inflammation.</strong> GSK-3 inhibition dampens NF-&#954;B signaling, one of the master switches of chronic inflammation. Given that low-grade chronic inflammation &#8212; sometimes called &#8220;inflammaging&#8221; &#8212; is now considered a primary driver of age-related disease, this mechanism alone could explain much of the mortality benefit.</p><p><strong>Metabolic regulation.</strong> Improved insulin signaling and glucose regulation, with epidemiological links to lower rates of obesity and diabetes in populations with higher lithium in drinking water.</p><p><strong>Autophagy promotion.</strong> Enhanced cellular cleanup &#8212; the same mechanism targeted by rapamycin, currently the most studied pharmacological geroprotector. Lithium appears to promote autophagy through both GSK-3-dependent and independent pathways, making it complementary to mTOR-targeted interventions.</p><p>The breadth of these effects explains why the mortality signal is <em>all-cause</em> rather than disease-specific. A UK retrospective cohort study of 29,618 patients confirmed the association (Chen et al., <em>PLoS Med</em>, 2022).</p><p><strong>Clinical trials.</strong> A Brazilian randomized controlled trial by Forlenza et al. demonstrated that low-dose lithium (300 &#956;g/day &#8212; a fraction of psychiatric doses) stabilized cognitive decline in patients with amnestic mild cognitive impairment over 12 months, while the placebo group continued to deteriorate. A separate Brazilian trial using microdose lithium (300 &#956;g/day) in Alzheimer&#8217;s patients showed stabilized Mini-Mental State Examination scores over 15 months (Nunes et al., 2013).</p><p><strong>Head-to-head comparison.</strong> A 2022 network meta-analysis by Terao et al. compared lithium directly with aducanumab &#8212; the controversial Alzheimer&#8217;s drug approved despite ambiguous efficacy data, priced at $28,000 per year &#8212; and found low-dose lithium to be <em>more effective</em> at improving cognitive function scores.</p><p>A trace mineral available in drinking water outperformed a monoclonal antibody that took decades and billions of dollars to develop. Tomatoes, potatoes, cabbage, and legumes (particularly lentils) consistently show higher lithium content in food composition analyses. Leafy greens &#8212; spinach, kale, and chard &#8212; also contribute meaningful amounts.</p><p><strong>Grains and cereals.</strong> Whole grains retain more lithium than processed varieties. Brown rice, oats, and whole wheat are good sources, as lithium concentrates in the bran and germ layers that refining removes.</p><p><strong>Seafood.</strong> Oily fish &#8212; sardines, mackerel, herring, and anchovies &#8212; tend to accumulate more trace minerals, including lithium, from their marine environment. Shellfish, particularly oysters and mussels, are also notable sources.</p><p><strong>Nuts and seeds.</strong> Almonds, cashews, pistachios, and sunflower seeds provide lithium, along with other trace minerals.</p><p><strong>Fruits.</strong> Citrus fruits (oranges, lemons, grapefruit) and bananas contain modest amounts of lithium.</p><p><strong>Water.</strong> This is the variable most people overlook. Lithium concentrations in drinking water vary enormously by region &#8212; from near-zero to over 0.170 mg/L. Mineral waters from lithium-rich springs (historically sought out for their &#8220;healing properties&#8221; &#8212; now we may understand why) can be a meaningful source.</p><h3>Supplementation: What the Evidence Supports</h3><p>For those considering supplementation beyond dietary intake:</p><p><strong>Lithium orotate</strong> is the most widely available over-the-counter form. It crosses the blood-brain barrier more efficiently than other formulations, which is why it can be effective at much lower doses. Typical supplement doses range from 1&#8211;20 mg of elemental lithium per day &#8212; orders of magnitude below the 600&#8211;1,200 mg used in psychiatric treatment.</p><p><strong>Lithium aspartate</strong> is another supplement form, used in several of the clinical studies on cognitive function. The Forlenza trial that showed cognitive stabilization used just 300 &#956;g (0.3 mg) per day.</p><p><strong>Important distinctions:</strong> Lithium carbonate is the prescription psychiatric medication requiring blood monitoring and medical supervision &#8212; that is not what we&#8217;re discussing here. Lithium orotate and aspartate at low doses (1&#8211;20 mg) have shown a favorable safety profile in available research, though long-term, large-scale safety data remains limited. Trace lithium from food and water (micrograms per day) requires no monitoring and is part of normal human nutrition.</p><h3>A Note on Safety</h3><p>Low-dose lithium supplementation appears safe based on current evidence, but &#8220;low-dose&#8221; must be emphasized. Anyone considering lithium supplementation above trace dietary levels should consult with a physician, particularly those with thyroid conditions, kidney disease, or those taking medications that affect sodium balance. Pregnant or breastfeeding women should avoid supplementation beyond normal dietary intake.</p><div><hr></div><h2>Where Lithium Fits in the Geroprotector Landscape</h2><p>Geroprotectors &#8212; substances that target the biological mechanisms of aging itself rather than individual diseases &#8212; represent a fundamental shift in how we think about health and longevity. Lithium joins a short but growing list of compounds with credible evidence for life-extending properties:</p><p><strong>Rapamycin</strong> (mTOR inhibition) remains the gold standard, having extended lifespan in every organism tested. <strong>Metformin</strong> targets metabolic aging through AMPK activation. <strong>NAD+ precursors</strong> (NMN, NR) support cellular energy metabolism. And <strong>lithium</strong>, through GSK-3 modulation, appears to address neurodegeneration, inflammation, and metabolic dysfunction simultaneously.</p><p>What makes lithium remarkable in this group is its accessibility. Rapamycin requires a prescription and careful dosing. Metformin is prescription-only in most countries. But lithium exists naturally in your food and water &#8212; and the evidence suggests that simply living in a region with higher lithium in the water supply confers measurable longevity benefits.</p><p>This doesn&#8217;t mean lithium is a magic bullet. No single intervention is.
